Client Care Coordination

Whether your loved one is living at home, in an assisted living community, or in a skilled nursing facility, you can find comfort in knowing that his or her care is being overseen by an expert who has extensive knowledge about the availability, cost, and quality of local resources. Our Client Care Coordinator works closely with other professionals (i.e., Geriatric Care Managers, Social Workers, Liaisons, and the like) and makes regular visits to ensure that needs are being met, thereby offering reassurance to family members that their loved one is enjoying the highest quality of life.
